Flirting with chronic health risks? Use this tool to learn if this is so! Depending on your responses to questions in the assessment, a list of resources are offered that are tailored to decrease your risk for chronic disease.
The assessment is taken at your convenience, completed on your own time, can be taken as often as you wish, and is completely anonymous; you are not asked your name, address or any identifying information. Your age, sex and race are asked because those are factors that influence your health risks. You can e-mail the results to yourself, your doctor or just print them out.
